At On The Glow Med Spa, your safety and informed decision-making are our top priorities. Before any treatment is performed, we adhere to a strict consent process
Informed Consent
Prior to receiving any medical aesthetic procedure (including but not limited to Neurotoxins, Dermal Fillers, Microneedling, Lasers, or IV Therapy), you will be required to sign a specific Informed Consent form. This form details:
Consultation Requirement
A consultation with a licensed medical provider (MD, NP, RN, or PA) is required for all new patients before medical treatments can be administered. This ensures you are a medically suitable candidate for the procedure.
Right to Refuse
You have the right to refuse any treatment at any time prior to its administration. Our providers also reserve the right to refuse treatment if they believe it is unsafe, medically contraindicated, or if the aesthetic goals are unrealistic.
Minors
We do not treat clients under the age of 18 without parental consent and presence. Certain treatments are strictly restricted to adults aged 18 and over.
Photography Consent
We may ask for your consent to take before-and-after photographs for your medical record to monitor progress. With your separate, explicit permission, these photos may be used for marketing or educational purposes.
